Dr. Zak Taylor
Dr. Zak Taylor has been serving the Mayfield and Murray communities for many years. He began his career in 2014 by opening his first practice, Mayfield Family Dentistry, which quickly became a trusted name in the area. In 2018, Dr. Taylor expanded his practice by purchasing the office of Dr. Steven Farmer in Murray, which has since been rebranded as Elevation Dentistry. With years of experience, a passion for dentistry, and a commitment to providing his patients with the highest level of care, Dr. Taylor has become a well-respected member of the dental community in Kentucky.

Alysha Taylor
Alysha is a highly skilled Pediatric Physician's Assistant who graduated from the University of Kentucky. She dedicates one day a week to providing compassionate care to children at a local pediatric clinic. Her passion for the medical field led her to expand her expertise to her husband's dental practices (Dr. Taylor), where she excels in handling complex dental insurance claims. Alysha enjoys mentoring and training new team members, helping them to develop their skills and achieve their goals.

Dr. Jamie Greenwell
Dr. Greenwell graduated from the University of Kentucky College of Dentistry in in 2016. She went on to serve in the United States Army for seven years. Dr. Greenwell and Dr. Taylor purchased the Smile Center from Dr. Michael Warren in 2023. Jamie loves serving the people of Huntingburg and especially enjoys helping veterans get their smile back!

Dr. David Dardick
Dr. David Dardick grew up in Richmond, VA. It was there that he decided that he wanted to focus on becoming a dentist. He loves that he can make people better, not only medically, but also cosmetically. Improving patients’ smiles is not only a career for him, but also a hobby.
In 2011, he got his undergraduate degree in Chemistry at the University of North Carolina at Wilmington. At UNCW, he was on the rowing team his freshman year. He was heavily involved in intramural sports, including basketball, tennis, and volleyball. In his final year, he completed research in organic chemistry and graduated with summa cum laude.
In 2015, he completed his Doctor of Dental Medicine degree at the University of Kentucky. Finishing near the top of his class, he was given an award by the department of periodontics for his proficiency in the field. After graduating from dental school, he was selected for a one-year postgraduate residency by East Carolina University, where he received valuable experience in root canals, implants, and cosmetic dentistry. While in his residency, he helped open up a dental clinic in Brunswick Co, NC.
Since 2016, Dr. Dardick has been in private practice. Since graduation, he has gained additional certificates in Invisalign, surgical implantology, smile design/esthetic veneers and crowns, sleep apnea, endodontics (root canals), and botox/fillers. Dr. Dardick is dedicated to improving patients’ smiles any way that he can. He takes pride in being one of the top providers in the area.
In his spare time, Dr. Dardick likes traveling, yoga, snowboarding, and exploring new restaurants and bars.

Dr. Orr is not only a proud Murray native, but also a highly skilled and compassionate dentist with a passion for serving his community. With a degree from the University of Kentucky College of Dentistry and years of experience in the field, Dr. Orr brings a wealth of knowledge and expertise to his practice. He is dedicated to providing the highest quality dental care to his patients and is committed to staying up-to-date with the latest advancements in dentistry. As a Murray High School graduate, he has a deep understanding of the needs and values of his hometown and is honored to be able to give back to the community that has given him so much.
